Collaborative Partners Information Request!

Good morning collaborative partners. I am looking for information on who is currently providing Nursing Home Transition Services? Please email nhabecker@berksencore.org if you are currently providing these services.
Secondly, I am looking for information on collaborative partners that are providing services that when a senior calls or those with disabilities into your organization, they would receive a call back from a peer that is also a senior or a peer with a disability? Again, please email nhabecker@berksencore.org to let me know.
