  • Self-Determination Housing Project of Pennsylvania, Inc. – Self-Determination Housing Project of Pennsylvania works “to promote self-determination and control in housing for persons with disabilities and older adults in the Commonwealth of Pennsylvania.” As a statewide agency, SDHP plays four key roles in this effort: as educator, policy maker, technical assistance provider and initiator of demonstration projects. Regional Housing Coordinators provide technical assistance to public and private housing providers to navigate and better understand social service systems. RHCs also work directly with social service entities including nonprofits, local Area Agencies on Aging, LINKs to Aging and Disability Resources, Centers for Independent Living, and numerous others to help them understand and identify the housing needs of their consumers/clients and bring those needs to housing providers. RHCs provide support to caseworkers, nursing home transition providers, and others that deliver services to older adults and people with disabilities to improve their understanding of housing systems.
